I don't know why they would include promethazine, most commonly used as an antiemetic, in cough medication. It would only intensify the CNS depressant effects of codeine.

Promethazine is an antihistamine. Codeine naturally helps suppress cough for obvious reasons. These two together can be very effective in helping someone with a cold/flu get rest since it can be difficult to sleep with a stuffy nose and constant cough. Sizzurp/purple drank has been around for several years. As the link says some kids just come in to drug stores and drink an entire bottle of robitussin Dm to try and duplicate it but its not the same. Most times I've seen the seizure it's with sprite and jolly ranchers.
