I would think if that's the reasoning, then why should he get treated at all? He was leaving a treatment facility, yes?

 

If he IS receiving treatment, then I would think his body and his immune system would need to be in top-notch shape. Smoking works against that.

True. But to a long time smoker, the quality of life will go down. He will always crave a cigarette until he dies. It is sad, but it is true. It is a trade off. If he has a chance of beating the cancer or a significant increase in life expectancy, I concede to your point.
